Quitting Services for the Uninsured
Through a grant from the Utah Department of Health Tobacco Prevention and Control Program, participating Association for Utah Community Health (AUCH) community health centers offer Zyban and Chantix free of charge to their uninsured clients.
The client’s community health center primary care provider prescribes Zyban or Chantix. Prescriptions from other healthcare providers cannot be filled for free through this program.
The client fills the prescription at a Smith’s Pharmacy or the community health center’s in-house pharmacy at no charge to them.
Each client is eligible for up to 12 weeks of Zyban or up to 24 weeks of Chantix per year.
Eligibility
To be eligible for these services, the client must be uninsured, a tobacco user, and an established client with a primary care provider at a participating community health center.  If you are uninsured but do not currently go to a community health center for your primary care, you must become a client of the community health center for all of your primary care needs (not just for tobacco cessation help) before you will be eligible to participate in this program. You are encouraged to contact one of the clinics listed below to schedule an appointment, but please be aware that you may have to wait before one becomes available because some community health centers may already have too many established clients to accept new patients at this time.
